Geriatric Services Hubs (GSH) are complex interventions with multiple interacting components, involving different organisational partners providing different levels of care to frail older persons living in the community. In this multi-site evaluation study involving hospitals, primary care providers and community agencies, the evaluation team seeks to provide an assessment of the implementation of the pilot GSH programme, the quality of care and health outcomes, the client and caregiver experience, and the impact on service use and cost.<\/p>\n
